Application Instructions

An affidavit will be attested by the Embassy for the purpose of Certify Residence, Renew/Obtain Driving License, and Opening Bank Account in Thailand and the applicant should be a Sri Lankan citizen, and should bring all relevant documents to the Embassy in person

Documents Required:

Duly completed application form for apply for an affidavit for Certify Residence, Renew/Obtain Driving License, Opening Bank Account (Download)
Original existing Sri Lankan Passport with valid visa to stay in the country of Residence
Original Sri Lanka National Identity Card (N.I.C) or driving license

Payment Details

Each copy of the document will be charged separately.
Payment must be made ONLY in cash (Credits/Debit Cards are not acceptable)
